Pagenstecher studied medicine in Giessen, Heidelberg and Würzburg and went to Wiesbaden in 1849/50, by then having obtained his doctorate. Here he passed his first state medical examination. He travelled to Paris for training and further education and specialized in ophthalmology.
In 1852, he returned to Wiesbaden and was employed as a medical assistant at the civil hospital (Bürgerhospital). In 1853, he set up a private ophthalmology practice in Kirchgasse, from which the ophthalmology clinic developed. At the same time, Pagenstecher ran a "poor people's eye clinic", which moved into the semi-detached house at Taunusstrasse 59/Elisabethenstrasse 1 in 1857. He was appointed court councillor in 1857.
Pagenstecher was awarded the Nassau, Prussian, Russian and Brazilian orders of merit for his achievements, including intracapsular lens removal with the so-called Pagenstecher spoon and his interventions for glaucoma and cataracts.
He was buried in the old cemetery on Platter Straße.
