After studying medicine and gaining his doctorate, Guradze worked as an assistant to the professor of orthopaedics, Albert Hoffa (1859-1907), at the Würzburg Orthopaedic Clinic.

In 1907, Guradze opened a private orthopaedic clinic in Wiesabaden with an attached workshop at Mainzer Straße 3, which he ran until his death. He also had the necessary aids manufactured here. He was particularly committed to treating children who suffered from physical disabilities, e.g. as a result of rickets and tuberculosis.

In 1909, Guradze was one of the founders of the "Verein für Krüppelfürsorge e.V. Wiesbaden Frankfurt" together with the orthopaedist Dr. Alfred Stein and other citizens, such as the later first chairwoman, Elisabeth König. Their aim was to help disabled children from poorer families in particular. In 1910, a provisional home was opened in a villa at Mozartstr. 8, which Guradze ran on a voluntary basis together with Dr. Albert Stein and the matron Freiin Hedwig von der Goltz. With the help of a donation, a new building for the home was constructed after the First World War in the vicinity of the Luther Church, which Guradze presided over and which was named "Alfred-Erich-Heim" after the founder's sons.

Guradze acted as a "state cripple doctor" and was a member of the German Society for Orthopaedics. He was responsible for publications on topics such as the treatment of gunshot wounds, problems with the stump of the thigh, scoliosis operations and diseases of the foot. After his death, the orthopaedic surgeon Joseph Borggreve took over the management of Guradze's private clinic and the Alfred-Erich-Heim.
