Altstadt-Grupp attended the Mainz Conservatory from 1908 and studied at Dr. Hoch's Conservatory in Frankfurt am Main. After the First World War, she moved with her parents from Mainz to Wiesbaden.
She worked here as a concert pianist and music teacher from 1923. She could also be heard as a pianist in radio broadcasts. She gave piano concerts at the Kurhaus under the conductors Carl Schuricht, Dr. Helmuth Thierfelder (1897-1966) and Hans Vogt (1911-1992). Altstadt-Grupp also worked as a music editor for several newspapers and magazines.
In her second marriage, Altstadt-Grupp married the civil servant Eugen Grupp. In 1952, she founded the Wiesbaden chapter of the GEDOK (Gemeinschaft Deutscher und Oesterreichischer Künstlerinnenvereine aller Kunstgattungen), which had been founded in 1929 by Ida Dehmel (1870-1942), and three years later an artists' hoard, which she managed and administered until the end of her life.
